Return to Work (Employees)

• Please provide Release to Return to Work documentation from your medical provider prior to placement back on the schedule to your manager as soon as possible. The attached Fitness for Duty Certification may be used.
• Please contact Sedgwick Leave & Disability via phone (866-431-8484) on the date you return to work or the next day so the leave claim can be updated.
• Failure to report the return from leave may result in payroll and benefit implications.
