Eligibility

  • Hourly employees who have been Full-Time for at least 1-year from most recent Full-Time status start date.
  • Salary employees who are Full-Time, 1st of the month following date of hire.

Policy

Waiting Period: The first week of leave is unpaid and is referred to as the “waiting period” (7 calendar days). During this time you may use up to 40 hours of accrued and unused Paid Time Off (cannot borrow PTO for this purpose).

An employee’s health care provider will be required to submit separate medical documentation to support their application for Short-Term Disability (STD) benefits. Employee’s own medical condition must be certified by their healthcare provider to qualify for STD benefits.

Employees are not eligible to concurrently receive STD benefits and workers’ compensation benefits.

Hourly 60% of base pay; up to $500 per week Benefit from day 8 - 180; benefits may be offset by State disability
Salary 100% of base pay Benefit from 8th day; up to 12 weeks; benefits may be offset by State disability
60% of base pay Benefit from weeks 13 through 26; benefits may be offset by State disability

You must submit your PTO request to your manager.

  • Hourly employee, your manager must enter your PTO hours into Kronos.
  • Salaried employee, submit PTO request via Kronos.

In the case of maternity leave, if you elect to continue your leave after STD ends (referred to as the “bonding period”), you may also substitute accrued and unused PTO to maintain pay (cannot borrow PTO for this purpose).

Leave type and leave approvals will be managed by the EssilorLuxottica Human Resources Service Center in partnership with Sedgwick.

IMPORTANT: 

Documentation is time-sensitive.

  • Employee must be aware that not adhering to medical due dates will delay and/or cause denial of Short-Term Disability/Leave of Absence.
  • Failure to provide certification to support time away from work can result in period being addressed under the department attendance policy or termination of employment.
