Eligibility

To be eligible for FMLA leave, employees must have worked for the Company for at least 12 months and have worked at least 1,250 hours in the 12 months prior to taking FMLA leave.

Policy

The FMLA provides up to 12 weeks of unpaid, job protected leave during a single 12-month period to eligible employees for the following reasons:

  • Birth and care of a newborn child of the employee
  • Adoption or foster care placement of a son or daughter with the employee
  • Care for a spouse, domestic partner, son, daughter, or parent with a serious health condition
  • Medical leave when the employee is unable to work because of a serious health condition
  • Qualifying exigencies arising out of the fact that the employee’s spouse, son, daughter, or parent is a service member on federal active duty, or has been called to federal active duty status, and has been deployed to a foreign country (applies to National Guard, Reserves, and Regular Armed Forces).

Family Medical Leaves are unpaid. Employees may use accrued and unused PTO, vacation or sick time concurrently.

Leave type and leave approvals will be managed by the EssilorLuxottica Human Resources Service Center in partnership with Sedgwick.

IMPORTANT: 

Documentation is time-sensitive.

  • Employee must be aware that not adhering to medical due dates will delay and/or cause denial of Short-Term Disability/Leave of Absence.
  • Failure to provide certification to support time away from work can result in period being addressed under the department attendance policy or termination of employment.
