• Is the employee aware of the attendance standard?
  • Did the employee know they were scheduled to work?
  • Did anyone try to contact the employee? If so, who and what time was the employee contacted?
  • Is there any reason why the employee didn’t show up to work?
  • What is the employee’s perspective?
  • How far in advance are your schedules being posted?   Were there any changes to the schedule?
  • How have you addressed similar attendance issues in the past with this employee or other employees?
  • If a NCNS, did the employee call in at any point during their scheduled shift?  If so what time?
  • Was the shift a store meeting?  If so, did other employees fail to attend the mandatory meeting as well?
  • Did the employee say anything to any other Luxottica employees on duty prior to leaving before the end of their last shift?
  • If late store opening, what time did the store open and who opened the store? What reason did the employee provide for the late store opening?
  • Does the Employee have any documented coachings or FCARs in their file in the past 12 months?  If so, what was it for and when was it delivered?
  • What would you like to see happen or do about this situation?
  • Did you partner with your Regional Manager?  Do they support the action you want to take?
