EssilorLuxottica is committed to keeping employee health and safety a top priority. In accordance with certain local, state and federal COVID-19 vaccine mandates, we now require applicable employees to provide documentation of fully vaccinated status as a condition of employment.  For employees who believe they qualify for an exemption to a government mandated COVID-19 vaccination requirement or who would like to request another accommodation, we have created a process to submit your request for consideration for a medical or religious accommodation.

To expedite requests for accommodation, employees who believe they qualify for an accommodation based on their own underlying medical condition, or due to their sincerely held religious belief (note: social, political, or economic philosophies, or personal preferences, are not "religious" beliefs under the law) should start by reviewing the Request for Accommodation: COVID-19 Vaccination Exemption Process flow chart and then printing and completing the applicable accommodation request form:

Once the employee has completed the appropriate form(s), they should submit a Consultation Request via the Employee Relations e-Service portal on My Personal Desk and attach the form along with their name and contact information, and a member of the Human Resources Department will contact them to discuss their request, as well as next steps in the interactive process.
